For sponsorships that support the continuation of an existing program, sponsor recognition will begin in November 2012 (Awareness Month) and continue through October 2013. For sponsorships that support the development and implementation of a new program, the sponsor will be recognized from the date of launch for a period of twelve months. Sponsors of printed resources will be recognized until the resource is exhausted.

2012 Congressional Luncheon

A constituent visit at the 2009 Congressional Luncheon was directly responsible for the introduction of the Tom Lantos Pulmonary Hypertension Research and Education Act in the Senate. This is just one example of the many ways the Congressional Luncheon has served as an invaluable tool for connecting the PH community with Senators and Representatives. Luncheon speakers will educate Members of Congress and/or their staff about PH from the patient, caregiver and physician perspectives and share information about PHA’s bill. Attendance is built from the grassroots, with PH community members encouraged to extend personal lunch invitations to their Members of Congress. Bill sponsors Kevin Brady (R-TX) and Lois Capps (D-CA) will also be invited to play a visible role in the event. Last year’s Congressional Luncheon had significant representation from elected officials, Congressional staff members, patients and patient supporters.

Insurance Advocacy and Education Program

Although the pulmonary hypertension field is growing in its ability to provide effective therapies, a single insurance payer trying to cut costs can leave patients stranded without treatment. PHA’s Insurance Advocacy and Education Program focuses on ensuring that patients have access to the treatment they need to improve their quality of life by empowering patients to be self-advocates and mobilizing leading medical experts in the field to respond to changes in policy and regulations.
With industry changes due to healthcare reform well underway, 2013 promises to be an active year for PH patients. Rulings similar to the one in Alabama limiting the number of therapies a patient can receive are projected to emerge in other states as local governments try to cut costs. PHA’s Insurance Advocacy and Education Program is invested in bringing the right people to the table at the right time to keep the PH community fighting our treatment access battles. Sponsorship of PHA’s insurance related activities supports not only its advocacy efforts, but also quarterly webinars, two unbranded ads per year in PHA’s Advances in Pulmonary Hypertension journal, the development and distribution of Insurance-in-an-Instant planning guides for Support Group Leaders, ongoing content development and promotion of the Online Insurance Guide, monthly Coverage Connection e-newsletters and many other print and online educational resources.

When new advocates join PHA’s grassroots advocacy network, the 435 Campaign, they begin to receive campaign updates and calls to action via email. However, PHA knows that effective member engagement goes beyond emails; members also receive a personal email or phone call welcoming them to the campaign. The Advocacy Guide is the next step in member engagement: a tool that provides each new advocate with a user’s guide on everything from writing effective advocacy emails to planning Congressional visits for their Support Group. Sponsorship covers the development and distribution of both a digital guide and 500 printed guides.

Support Group Welcome Brochure

As a PH Support Group leader, it can be difficult to strike a balance between devoting special attention to newly diagnosed patients and new support group members while simultaneously providing for the needs of the larger group. The Support Group Welcome Brochure allows leaders to easily provide an ample introduction to PHA and their support group without ignoring the continuing needs of long-term group members. This brochure explains the purpose of the support group, helps new members know what to expect, provides access to other valuable PHA resources and provides a way for new members to connect with PHA. PHA plans to design, print and distribute 10,000 brochures to support groups across the nation.

Support Group Leader Manual

The Support Group Leader Manual is a valuable tool for both new and experienced leaders of support groups throughout the PH community that provides comprehensive instructions from the first steps of planning a meeting to resources for the ongoing management of group dynamics.
This year, PHA will be doing a complete redesign of the current manual, breaking it into three segments so leaders can get exactly what they need, whether general information, specific guidelines for setting up an initial meeting, or ongoing management of a group. Copies will be mailed to all 300 support group leaders, as well as new leaders as they are oriented.

NEW! PHA Special Event Toolkit

A key part of PHA’s success is its community’s enthusiasm for fundraising through special events. The Special Event Toolkit will be a digital and printed resource containing helpful tips, resources, timelines and examples to assist special event planners in hosting a PHA Special Event. In addition to general event planning guidance, and specific information on hosting a PHA Signature Event such as Fun Walks/Runs for a Cure and Periwinkle Parties, the toolkit will include a section about incorporating the Sometimes it’s PH: An Early Diagnosis Campaign into an event.
The kit’s easy-to-understand, structured how-to information and templates will ensure that event planners, regardless of prior fundraising experience, can host successful community-based PHA Special Event fundraisers. The sponsorship will cover the distribution of 100 printed toolkits and 500 flash drives containing a digital copy of the toolkit, including editable template forms compatible with Microsoft Office.

Family Support Program

PHA’s Kerry Bardorf Family Support Program provides education and support to family members of children and adults with PH. Over the past two years, the program has introduced monthly telephone support groups and informational webinars, enhanced family programming at PHA’s patient education events, offered a teen social network and developed information packets geared to caregivers, parents and pediatric patients. This year, PHA will offer the following major initiatives:
- Ongoing distribution of Pediatric Resources for Family and Friends and PH Handbook for Families (geared toward pediatric patients);
- The creation, design and distribution of a new booklet for the children of PH patients, explaining adult PH in kid-friendly language;
- Continuation of PHA’s monthly telephone support groups for parents and caregivers, with regular webinar offerings; and
- The completion and distribution of a Transition Guide to help teens transitioning from pediatric to adult care, and the update and distribution of the School Resource Guide to help parents explain their child’s needs to schools.

Psychosocial Support Program

In response to patient and caregiver feedback about the prevalence of denial, depression, anxiety and stress among those affected by PH, PHA recently developed population-specific Coping with Pulmonary Hypertension guides for newly diagnosed patients, long-term survivors, parents, caregivers and teens.
In addition to offering the guides on the website for download, PHA plans to print 1,000 of the Newly Diagnosed Patients and Long-Term Survivors guides to distribute directly to patients, PH centers and associated disease organizations. With the help of a professional with a background in social work, PHA will continue training its community leaders in how to address psychosocial issues that they encounter as they offer support through the PH Email Mentor program and Patient-to-Patient telephone support line.

The “10 Things to Do When You, Your Child, or Someone You Love is Diagnosed with Pulmonary Hypertension” card is a quick, easy-to-carry reference that helps patients and caregivers understand the basic next steps after being diagnosed with PH. This year’s sponsorship includes printing 30,000 copies of the original “10 Things to Do” card and 5,000 copies each of three variations, including “10 Things to Do in an Emergency,” “10 Things to Do When Traveling” and “10 Things to Do to Prepare for a Doctor’s Appointment.” The cards will be distributed to PHA support group leaders and special event planners, included in Envelope of Hope packets, and made available at patient education events.

Envelope of Hope Program

PHA’s Envelope of Hope Program is a gateway to the newly diagnosed patient community that provides patients and their families timely and targeted access to information about PH, treatment and coping. The Envelope of Hope offers information and resources for patients, while Resources for Family and Friends provides information and resources for family members of PH patients. PHA currently receives requests for 25 to 50 packets per week. As more patients are diagnosed, PHA has received a growing number of requests for information in Spanish. This year, PHA plans to identify a list of critical resources to translate and offer through the Envelope of Hope and Resources for Family and Friends packets. In addition to accepting orders through an online form, PHA also distributes Envelope of Hope referral postcards to PH-treating medical professionals, event planners and support group leaders to give to patients they know.

"PH and ..." Brochure Series

In 2010, PHA created a series of brochures designed to reach medical professionals and patients through the diseases that may lead to PH, with the goal of broadening their understanding of PH and the need for screening. These brochures include:

Specialty Pharmacy Fliers

Every month, specialty pharmacies send a medication package to their patients, reaching PHA members as well as non-members. Accredo, CVS Caremark and CuraScript, understanding their role in providing patient education and awareness and as part of the Corporate Committee, have provided PHA access to these medication packages. Each of these monthly mailings includes a different flier, allowing PHA to reach tens of thousands of patients with information on how to handle emergency situations, how to connect to a local support group and how to access PH resources online. This flier is often a patient’s first introduction to PHA and its community of information, connection and hope.

Now in its sixth year of production, the Community of Hope Calendar serves as a daily testament to the vibrancy of the PH community. The Community of Hope Calendar, featuring photographs of and unique artwork produced by talented members of our PH community, was honored with the 2011 Apex Award of Excellence in the Calendars, Posters and Certificates category. Two thousand calendars will be produced and distributed through PHA’s end-of-year holiday card mailing as gifts of appreciation to PHA’s vast list of VIPs, including event and program volunteers, Support Group Leaders, Mentors, the Board of Trustees and Scientific Leadership Council.

New to this year’s packet is a “Jump In!” section where patients and families get a sneak peak at new initiatives at PHA such as the Sometimes its PH: An Early Diagnosis Campaign.

PHA folders serve as an easy reference point for those looking for information about the disease, resources on living with PH, or ways to connect to the PH community. PHA Signature T-shirts

PHA Signature T-shirts are provided to special event organizers and volunteers as a benefit of hosting a Special Event fundraiser within their community. They are an effective way to promote solidarity among event planners and volunteers, while simultaneously raising awareness for PH. The Signature T-shirts allow PHA to promote awareness of PH, brand special events as PHA-supported activities, and spread the message of hope each time members of the community wear their shirts. PHA plans to produce 2,000 t-shirts.

11th International PH Conference and Scientific Sessions Save-the-Date

PHA’s 10th International PH Conference and Scientific Sessions brought 1,500 patients, caregivers and medical professionals together to learn from leaders in the field of PH and celebrate the hope that this gathering continues to inject into the PH community. Many attendees learned of the Conference by receiving a Save-the-Date postcard. With the 11th International PH Conference and Scientific Sessions coming up in Indianapolis, Ind. in 2014, PHA plans to design, print and mail 20,000 Save-the-Date postcards so the PH community can plan ahead and look forward to attending. The cards will be mailed to targeted lists from PHA’s database and included in attendee bags at four PHA on the Road: PH Patients and Families Education Forums in the spring of 2013. Thanks in large part to the PHA Medical Education Fund, medical professionals, patients and caregivers are coming in contact with PHA in greater numbers than ever before. While PHA has many brochures for individual programs and services, this brochure answers the simple but important question, “What is PHA?” Due to the brochure’s popularity, PHA is seeking to reprint the brochures for continued distribution over the next year.
Brochures will be made available at PHA on the Road forums, medical and patient conferences and Building Medical Education in PH events. Additionally, PHA will provide copies to special event hosts, support group leaders, and workplace fundraisers, and include copies in media kits and general inquiry packets. PHA plans to print and distribute 15,000 brochures.

2012 End-of-Year Matching Fund

With matching gifts from members of the PHA Corporate Committee, the End-of-Year fundraising campaigns have been immensely successful. When donors know that the impact of their gift will be doubled or tripled thanks to matching gifts, their motivation to support PHA increases considerably. In 2011, despite continued economic hardships, the greatest number of donors yet rose to the occasion and made gifts totaling more than $100,000 to PHA for a campaign of more than $125,000.This unrestricted giving covers a significant portion of the costs of essential initiatives designed to advance PHA’s mission, like the PHA Patient-to-Patient Support Line and Pulmonary Hypertension: A Patient’s Survival Guide. PHA is again seeking sponsors to match its constituents’ donations for the 2012 campaign. The match can be dollar-for-dollar, or two or more to one – each company may set its own challenge.

International Opportunities
Global Program Connections

In order to initiate global awareness of pulmonary hypertension and support for patients, PHA has worked hard to encourage the growth and networking ability of PH associations around the world. Exhibiting PH resources and distributing Advances in Pulmonary Hypertension, PHA’s medical journal, at international medical meetings each year has proven effective in building a professional network for clinicians and researchers (PHCR) and expanding the reach of PH knowledge to new populations. PHCR has 60% more international members in 2012 than in 2009-2011 combined, and that number continues to grow. And with more than 600 leading physicians in the field as members of PHCR, PHA is anxious to engage those who travel and speak internationally with its message.
This sponsorship will create increased opportunities for face-to-face connections abroad by supporting:
- Exhibits and program materials at international conferences;
- Development of materials that PHA medical leaders can integrate into existing presentation opportunities; and
- Expanded international membership in PHA’s professional network for physicians and researchers, PH Clinicians and Researchers (PHCR).

International Survival Guide Distribution
$2,500 co-sponsorships
PHA’s comprehensive Pulmonary Hypertension: A Patient’s Survival Guide is a “soup-to-nuts” book for patients living with PH. Over the years, PHA has received requests from individuals and PH associations in other countries to translate the Survival Guide into their native language. To date, the Survival Guide has been translated by medical experts fluent in English into Chinese, Japanese, Spanish, Farsi and Korean. PHA’s goal is to distribute copies of the Survival Guide to every association with an explanation of how the translation process works. The translations, which undergo medical review, benefit not only patients living in that country but also patients in the U.S. whose primary language is not English.
Sponsors will be recognized in the letter that accompanies Survival Guide mailings, on PHA’s Awareness Month website and in the quarterly newsletter Pathlight.
International Seed Grant Program
$30,000 co-sponsorships
PHA’s International Seed Grant Program has been instrumental in the development and growth of global PH associations. The formula is simple: applicants must demonstrate to a peer review panel that — with a small infusion of cash — they have a plan to more rapidly accelerate the growth and effectiveness of their association and are willing to share their lessons learned as a best practices across the globe.
Every year, the review panel awards up to three grants of a maximum of $10,000 each or six grants of up to $5,000 each. The grant proposals are reviewed by seasoned leaders who have helped to build PH associations in the U.S. and other nations, including at least one past International Seed Grant winner. Their role goes beyond peer review to mentoring, and they often help applicants refine their ideas to increase the likelihood of success.
