Conference Pre-Registration Closed
$25 fee for On-Site Registration
Conference pre-registration ended June 14. We will not be able to process any more registrations in order to prepare registration packets in time for conference. You must now register on-site at the conference, but will incur a $25 processing fee. More info on conference agenda
Conference on-site registration fees:
- Patient - $175
- Companion/Family Member* - $275
- Nurses/Therapists/Pharmacists - $320
- Physician/Corporate - $420**
- Children (5-12) - $150
- Children (under 5) - free
All attendees must be a PHA member. If you need to renew your
membership or become a new member you need to do so during the conference registration process. If you are unsure of your
membership status please call Jeannine at 301-565-3004 x108. Complimentary

Membership Options (must be a current member to attend):
- PHA Membership, individual - $15
- PHA Membership, family - $35
- PH Clinicians & Researchers (PHCR)**- $135

- PH Resource Network (non-physician medical professionals) - $35
*The Family Member conference registration rate includes family of medical professionals
in attendance.
**Current and new members of the PH
Clinicians and Researchers will receive a $100 discount on
conference registration. New
members applying for PHCR must fill out an application AND
make payment through the conference registration. The $100 conference
discount will be refunded upon approval of their application.
Scholarships
To obtain Conference scholarship information and an application,
please go to the Scholarship section of the Conference website
or call 301-565-3004 to have the information mailed to you. A
patient-led committee will exercise the award process based upon
available funds.
Registration of Children and Teens
Contact Parent Coordinator to Help Ensure Adequate Resources for Children
Laurie Jeter, our PHA Parent Coordinator, is working to acquire the resources we need for children and teens at conference. If you have a child ages 3-12 or teen ages 13-18 attending conference who will be participating in the children and teens activities, please contact Laurie at mjeterl@aol.com or 952-380-4999 with your children’s names, ages, and if they will be participating in the Saturday outing to the science center. Please help us prepare enough resources for your children by contacting her as soon as possible. Laurie has a lot of fun and exciting activities planned. Payment Options:
On-site line registrations can be made by check or with Visa, MasterCard,
or AmericanExpress credit cards.
