Conference Pre-Registration Closes June 10, 2008
Registration fee includes all Conference sessions; dinner on Friday; breakfast, lunch and dinner on Saturday; breakfast on Sunday; coffee breaks, Conference bag & t-shirt. Hotel reservations must be made directly with the Hilton Americas and are not included in the registration fee.

- Companion/Family Member - $250
- Children (3-12) - $125
- Children (under 3) - Free
- Nurses/Therapists/Pharmacists - $295
- Physician/Researcher - $395
- Medical Student/Resident - $250
- Corporate (including medical professionals) - $395
All attendees must be members of PHA. If you need to renew your membership or become a new member, you can do so during the Conference registration process. If you are unsure of your membership status please contact Andrew at the PHA office at member@PHAssociation.org or 301-565-3004 x116. - PHA Membership, individual - $15
- PHA Membership, family - $35
- includes family of medical professionals as well as patients
- PH Clinicians & Researchers (PHCR) - $135
- Members of PH Clinicians and Researchers (PHCR) will receive a $100 discount off their conference registration fee.
- New members applying for PHCR must fill out an application AND make the full registration payment of $395. The $100 conference discount will be refunded upon approval of the application.
- PH Resource Network (non-physician medical professionals) - $35
Because the online registration form requires payment, if you are a Conference scholarship recipient or have a pending scholarship application, you must register using the Print Conference Registration Form. Please contact scholarship@PHAssociation.org with any questions.

Cancellations made before April 30, will be refunded in full, less a $25 processing fee. Between May 1 – June 10, cancellations will be refunded half the registration fee, less a $25 processing fee. Cancellations made after June 10 will not be accepted without a physician’s letter. Cancellations must be made in writing.
If you cancel your registration, you must contact the hotel to cancel your room reservation or the hotel will charge your account. |
