Apply for a Conference Scholarship
Application Deadline: April 30, 2010
PH patients diagnosed after March 1, 2010 have until May 30, 2010 to apply.
Before completing an application, all applicants are advised to read the following:

Applicants are strongly recommended to use the online application. A few things to keep in mind:
- Please note that partially completed forms cannot be saved.
- Be sure to double check all information before submitting your application. Once the application has been submitted it cannot be changed.
- If you do not receive a confirmation email, please contact PHA at Scholarship@PHAssociation.org to confirm receipt of your application.

Apply using the Paper Application
PHA is committed to assisting as many patients as possible through the Conference Scholarship program; however, this is dependent on the available resources in the Conference Scholarship Fund. We regret we are unable to support all who apply for scholarship assistance. A committee of patients and caregivers reviews applications and administers the award process.
