Conference Program Tributes (in honor of, in appreciation of, in memory of, etc) Due Tuesday
PHA makes space in the conference program for people to recognize those in the PH community special to them. These "tributes" can be purchased to honor your doctor, nurse, caregiver, patient, clinic staff, family, etc. Tributes cost $25 each and consist of a line in the program saying "In honor of _______ by _________." They can also say "in appreciation of" or "in memory of" and include a few words saying what the tribute is for.

A couple examples from last year's program: "In honor of Dr. Robyn Barst from the Carr family for 11 years of outstanding, compassionate care" "In memory of Rachel Hoyt by Laura D'Anna"

To purchase a tribute go to http://www.phassociation.org/Conference/2006/tributesubmit.html by Tuesday, May 9.
