PHCC Application Procedure

Applications for PH Care Centers accreditation are expected to become available in the first half of 2014. Centers will complete an application that will assess the program in five areas: Center Director, Center Coordinator, Program Staff & Support Services, Facility and Clinical Research.

After completion of the application, PHA will work with your center to coordinate a site visit by two experienced PH professionals: a physician and a center coordinator. This site visit is expected to take approximately one day. After the initial site review, preliminary assessment of the visit will be provided and the accreditation decision will be made by the full PHCC Site Review Committee.
