PHA PHriends Application Form

Thank you for your interest in becoming a PHA PHriend for one of the following: Patient-to-Patient Support Line volunteer, PH Email Mentor Program and/or Online Chat Leader.

Please note that PHA does not reimburse Peer Support Volunteers for incoming calls, internet usage, etc. The majority of our volunteers have a home telephone (landline) or cell phone that with unlimited minutes.

We require the Patient-to-Patient Support Line volunteers to be unemployed, as calls come in during work and evening hours. However, employment status does not disqualify you from participating as a PH Email Mentor or Online Chat Leader. If you are still interested, please fill out the form below for consideration.

All potential candidates will be chosen based on written application, telephone interview/s and participation in PHA’s Orientation Training Program. PHA’s demographic needs will determine who is chosen, as well. PHA's peer support program strives to be as diverse as possible in terms of diagnoses, age, sex, cultural background, lifestyle and physical abilities while staying within the program parameters.

PHA PHriends Job Description

Under the supervision of the Manager of Patient and Caregiver Services, and in conjunction with PHRIENDS, you will work with adults and teens affected by pulmonary hypertension to assist them to achieve their stated goals and alleviate current life challenges.

Duties include:
•    Assisting people affected by PH to achieve goals and problem-solve
•    Promoting services and programs of PHA
•    Interacting positively and respectfully with members of the PH community as a representative of PHA
•    Responding to inquiries in timely manner
•    Maintaining regular contact with PHA program staff
•    Attending initial, quarterly and annual trainings
•    Completing necessary program forms and documentation
•    Other duties as reasonably assigned

Qualifications
•    Must be at least 18 years old
•    Successful completion of PHA PHRIENDS training program or equivalent experience
•    Ability to work with a high degree of independence, including good judgment and problem solving skills
•    Have effective strategies for dealing with stress
•    Ability to work cooperatively with other PHA PHRIENDS volunteers
•    Ability to be a role model to people affected by PH
•    Willingness to work with a culturally diverse population
•    Knowledge of available PH and PHA resources
•    Be empathetic, patient and supportive
•    Has been living with PH for 2+ years since diagnosis
•    Must be medically stable and able to carry out outlined job description
•    Experience using necessary technology for role (internet, email, Microsoft Word)
•    A commitment to the mission of PHA

Time Commitment
A one year commitment to the program is required with an additional time commitment outlined below. 
•    Variable, depending on assignment
•    1-3 hours/week PH Email Mentors
•    1-6 hours/week Online Chat Leaders
•    10-20 hours – the week you are on duty with the Patient-to-Patient Support Line

There will be a three month trial period to ensure a “good fit” between the PHA PHRIEND and PHA. Additionally, PHA reserves the right to terminate the PHA PHRIENDS relationship if the above duties are not appropriately fulfilled.
