
Tab 3: Medical Records
In this tab, create a timeline of your medical history, from childhood through the present day. Be brief,
but specific. Include surgeries, medical conditions and hospitalizations.
You can also use this section of your binder to track your PH lab work and diagnostic tests. Ask your doctors for copies of any blood tests you have had performed. Most doctors will keep each other informed of any important lab work, but it is always good to have your own copy. You can also ask for copies of the results of all of the studies you have performed, such as CT scans, MRI scans, X-rays, echocardiograms, EKGs, ultrasounds, pulmonary function tests and sleep studies.
Track your hospitalizations, right heart catheterizations and other procedures using this log. Print out multiple sheets as necessary.
Keeping a complete and accurate family medical history can be useful to you and your medical team. Use this worksheet to record important medical information about your biological relatives.
